The Lady in the Jukebox was first performed as a musical play directed by Beau Decker. It was first written as an autobiography. The play was based on actual characters and events. Larry Scarinzi created the artwork for the characters. Martha Quiroz created the cover design. Sheila Greer wrote all the musical numbers, with Haim Cotton doing the arrangements. The first premier took place November 11, 1999, at the Producer's Club Theater in New York City. The original cast in order of appearance: Mary Lapegna as "Little One" Bill Herbert as "El Presidente" Terry Boddie as "Timebomb" and "George" John Gammon as "Magic Man" Sheila Geer as "The Lady in the Jukebox" Peter O'Hara as "Conor McClaronon" Genevieve Fort as "Angel" Doug Champigny as "Dave" Doli Grace as "Matilda Madison" Stephanie Mara as "Little Lady"
